Change fill in the form of 4-FSS will be introduced amendments to the Order of the Ministry of Health and Social Development of Russian Federation № 156n on February 28, 2011. These changes will take effect as early as the period of 2011. Thus, the document indicates which sections in the form of a 4-FSS should be filled without fail: cover sheet table number 1, "Calculations on compulsory social insurance against temporary disability and maternity," Table number 3, "Calculation of the base for calculating premiums," Table № 6 "base for calculating premiums," table number 7, "Calculations on compulsory social insurance against industrial accidents and occupational diseases."
